Request for Religious Exemption from Influenza Vaccine Requirement

Employees (faculty and staff) may contact Duke Access and Accommodation Services (DAAS) to request religious accommodations. DAAS will facilitate the process of identifying what, if any, reasonable religious accommodations may be available.
